Please include your name (and the patient's name if you are not the patient), daytime telephone number(s) where you may be reached, reason for the appointment, and preferred days and time of days you would like to come. Our scheduler will be happy to call you back or reply by e-mail confirming an appointment time. Please do not use this to make same-day or urgent appointments. Do not include confidential or extremely personal information. Although we utilize a secure e-mail system, we cannot guarantee that others may not hack into the system and retrieve your information.
